I certify that my vehicle and trailer (if applicable) are in safe, working condition, has sufficient fuel, have working lights, have spare tires and working jacks. Safety chains connect trailer and vehicle.*

I will leave a spare trailer key, incase of trailer failure.*

I understand and acknowledge that I am authorizing Wild North Guides staff to operate and transport my vehicle (and trailer, if applicable) from the launch site to the designated take-out point. I acknowledge that this involves driving on public or forest roads, in potentially remote areas, and that there is inherent risk involved in this activity.*

I hereby release, waive, and discharge Wild North Guides, its owners and employees from any and all liability for loss, damage, or injury to persons or property — including my vehicle and trailer — resulting from the shuttle service provided. This includes, but is not limited to, scratches, dings, tire or mechanical failure, theft, or damage caused by road conditions or third parties.*
